PMDD is wildly misunderstood. By doctors. By sufferers. But what sufferers of PMDD aren’t failing at, is feeling its impact…deeply. PMDD can dominate nearly aspect of the patient’s life. Work, marriage, friendships, family dynamics. The mountain of symptoms that it brings leaves little room for joy. If you’re lucky, you get those little glimmers of sporadic joy, but never during the luteal phase. That’s why many compare PMDD to PMS, but it isn’t just PMS. It’s its evil cousin.
Spiraling every month on repeat while you're simultaneously clawing your way out from the singed ashes of self sabotage and the bridges you've burned …can be utterly exhausting.
But first… you aren't alone. And second, why is it even happening? And.. what is it??
Well, maybe it's PMDD. Or maybe even PME.
Let's explore both options and why it matters: Getting the label right guides treatment and can help prepare you for what to expect each month
PMS (milder)
-
You're snappy and crampy for a day or two, but still functioning at work/school. Maybe even some fatigue.
-
Once your period starts, you're fine. Minus those cramps that make you second guess womanhood.
PMDD (severe + cyclical)
-
Timing: 5–14 days before bleeding
-
Impact: inability to concentrate, calling out of work, fights, relationship difficulties, despair, panic, sensory overwhelm, irrational rage, aggression, combativeness, brain fog, extreme fatigue, irritability, self loathing, sensitivity to rejection and criticism, hopelessness, suicidal ideation and more
-
Relief: symptoms lift soon after bleeding begins
PME (premenstrual exacerbation)
-
This is when you have another condition (e.g., MDD, GAD, ADHD, auto immune illness etc). It's present all month, but worsens pre-period in your luteal phase. You may experience typical PMDD symptoms on top of the elevated symptoms from your other illnesses.
-
Medical Treatment often targets the base condition + timing tweaks (e.g., med adjustments during luteal-phase)
Quick self-check
-
Do symptoms vanish after bleeding starts? (PMDD clue)
-
Are symptoms present most days with a luteal spike? (PME clue)
This is the short and sweet version. I'm sure I'll be back soon to tell you more about the horrors of PME specfically, but in the meantime, feel free to browse these pages for more PMDD info.Â
