Short answer: absolutely

This isn't a fringe theory, though perhaps the way I initially came across it was by accident. There's a biological pathway between undiagnosed or poorly managed celiac disease and the luteal phase symptoms that define PMDD. Let me break it down.
What is the connection?
Celiac disease is an autoimmune condition where gluten triggers an immune response that damages the small intestine. Most people think of it as a digestive issue, but it's far worse than that. The downstream effects of that gut damage reach far beyond your stomach ...including directly into your hormonal, mental, and physical health.
For people with PMDD, three pathways are especially relevant:
- Estrogen dominance ā a damaged gut impairs the body's ability to eliminate excess estrogen. The liver processes estrogen, but it needs a healthy gut to excrete it properly. When that system is compromised, estrogen builds up. Estrogen dominance is a well-recognized driver of severe PMS and PMDD symptoms. - Nutrient malabsorption ā celiac destroys the villi in the small intestine, making it impossible to properly absorb magnesium, B6, zinc, iron, folate, and B12. Every single one of those nutrients plays a direct role in mood regulation, neurotransmitter production, and hormonal balance. Deficiencies in magnesium and B6 alone are known to be independently linked to worsened PMS and PMDD.
- Histamine intolerance ā a damaged gut produces less DAO (diamine oxidase), the enzyme responsible for breaking down histamine. Histamine naturally spikes during the luteal phase. Add a DAO deficiency on top of that and you get a full-body inflammatory response every single month that can result in symptoms that include but are not limited to: worsened anxiety, rage, brain fog, insomnia, and physical pain. Put those three together and you have a perfect recipe for PMDD.
Let's talk numbers...
Celiac disease affects approximately 1 in 100 people worldwide, but it's estimated that up to 83% of people with celiac remain undiagnosed. Women are diagnosed at much higher rates than men, but are also more likely to present with atypical symptoms ā meaning no obvious digestive complaints ā therefore making it easy to miss or be lost in a gaslit friendly medical system.
The overlap with PMDD is not well-studied yet, but the mechanistic connection is clear: anything that chronically disrupts estrogen metabolism, nutrient absorption, and histamine regulation is going to impact the luteal phase.
There are also over 250 documented symptoms of celiac disease. Many are PMDD symptoms like anxiety, depression, brain fog, fatigue, mood swings, suicidal ideation, joint pain and more.
a genetic aspect to think about
Research has identified that people with PMDD show abnormal gene expression in the ESC/E(Z) complex which is a group of genes that regulate how cells respond to estrogen and progesterone. In PMDD, this same complex overreacts to normal hormonal fluctuations.
Chronic inflammation (exactly the kind that unmanaged celiac disease produces) is believed to keep that gene complex in a state of dysregulation. When the inflammation source is removed (in my case, gluten), the system can finally begin to recalibrate.
Worth noting: having the celiac gene doesn't mean it will automatically activate. Environmental factors and more can flip that epigenetic switch.Ā
My experience
I lived with debilitating PMDD for decades. I was told to manage it. SSRIs, birth control, lifestyle tweaks and I just kept suffering because none of it touched the root cause.
When I finally went completely gluten free (not mostly, not gluten-friendly, but completely) my PMDD disappeared within six months. Not improved. Gone.
And when I accidentally consumed gluten, it came back, making my luteal phase a living nightmare again. Every time. Like clockwork. That pattern, repeated over and over, removed any doubt for me. You can only see the same results so many times before it clicks.
The proof wasn't just in the healing. It was also in the relapses.
Should you get tested?
If you have PMDD and have never been properly screened for celiac disease, it's worth asking your doctor about. A few things to flag:
- Standard celiac blood tests can produce false negatives, especially if you've already reduced gluten before testing. Read more about false negative celiac tests here.
- You can have celiac with no digestive symptoms at all while neurological and hormonal presentations are common and commonly missed.
- Non-celiac gluten sensitivity (NCGS) can produce similar downstream effects and is even harder to test for.
